– This expanded value-based kidney care partnership extends access to Strive’s innovative kidney care model to Humana Medicare Advantage HMO and PPO plan members living with chronic kidney disease (CKD) and end-stage kidney disease ( ESKD) in five states.
Transforming kidney care delivery
This agreement signifies a commitment to value-based care, an approach to healthcare that prioritizes patients’ health outcomes and quality of life. Through this partnership, Humana members in Indiana, Illinois, Kentucky, Michigan and northwest North Carolina will benefit from:
– Access to Strive’s value-based kidney care program.
– A dedicated interdisciplinary care team that collaborates with a member’s physician to provide holistic care to the person.
Services including:
– Medication management and care management.
– Planning access to dialysis.
– Coordination of transplants.
– Social services.
Building on a solid foundation
This new agreement expands on an existing relationship between Humana and Strive, who began offering value-based kidney care to Humana members in Indiana and Kentucky in 2020. The success of this first collaboration paves the way for access broader and improved care for Humana members across the newly included states.
